Health Plan Benefits

We understand that the need for a quality health care package is vital to our employees and their families’ well-being. Our goal is to offer an affordable health plan that includes wellness coverage for the whole family and encourage consumer awareness when it comes to using a prescription drug plan.  In addition, we offer supplemental coverage’s’ to protect you financially.

Eligibility for enrollment onto our plans is 30 days for experienced hires and 60 days for non-experienced, with coverage starting the first of the following month.  Eligible dependents and spouse are welcome.

Health Insurance – Anthem Blue Cross Blue Shield of Wisconsin. We offer a choice of three different health insurance plans – Gold, Gold Plus and the Blue Saver plan – All Plans offer in-network and out-of-network benefits as well as prescription drug coverage through Express Scripts.

Dental Insurance – Delta Dental. Our dental plan offers 100% covered routine diagnostic and preventative care which is designed to encourage prevention of serious dental problems. Your spouse and dependents are also entitled to receive benefits under the dental plan.

Vision Insurance – Superior Vision. Our vision plan is designed to help cover the expense of exams, lenses, contacts and frames.

Basic Life & AD&D Insurance – Administered by CignaA $25,000 Life/AD&D insurance policy is offered to all full-time employees with the company paying the full cost of the premiums.

Short Term Disability coverage is offered to all eligible full time employees with the company paying the full cost of the premiums.

Supplemental policies are offered for additional life/AD&D insurance and long term disability insurance through Cigna. An accident insurance policy through Allstate Insurance and critical illness insurance through Transamerica.

Employee Assistance Program (EAP) – Administered by Cigna. Whether your needs are big or small, your Life Assistance & Work/Life Support Program is there for you. Cigna can help you and your family find solutions and restore your peace of mind.

Pretax saving opportunities – We offer a FSA (Flexible Spending Account) and a HSA (Health Savings Account) to all eligible participants in our health plan. These allow you the option of pre-tax pay deductions for unreimbursed health expenses and child/dependent care expenses.
